CPT code 90785: Interactive complexity (add-on)

90785 is an add-on for interactive complexity: specific communication factors that make a session harder to deliver. It never stands alone. You add it to the intake (90791), individual psychotherapy (90832, 90834, 90837), or group psychotherapy (90853) when a qualifying factor is present, and the note has to say which one.

How it's counted
Added to a qualifying service
2026 Medicare rate
$14.70

When to use 90785

  • Communication among the people in the session (high anxiety, reactivity, repeated questions, or disagreement) makes the service harder to deliver
  • A caregiver's emotions or behavior get in the way of carrying out the treatment plan
  • A sentinel event, such as suspected abuse, leads to a mandated report and a discussion of it in the session
  • Play equipment, other devices, or an interpreter are needed because the client hasn't developed, or has lost, the language skills to communicate directly

What to document

  • Which interactive complexity factor was present
  • How it affected the session and what you did about it
  • The primary service it was added to, with that service's own documentation

Common reasons 90785 gets denied

  • Billed alone, or with a code it can't attach to, such as crisis psychotherapy (90839, 90840) or family therapy (90846, 90847)
  • Billed only because an interpreter or translator was used. CMS guidance says interpretation alone doesn't qualify
  • Added to every session for a client by default instead of when a factor was actually present

Billing 90785 by telehealth

The CPT code stays the same when you see a client by video or phone. For Medicare, the place of service changes. Use 10 when the client is at home and 02 when they are somewhere else, such as a clinic. Add modifier 93 when the session was audio-only. Commercial plans and Medicaid set their own rules, and many of them also want modifier 95 on video sessions, so check each payer's telehealth policy.

Medicare covers mental health telehealth in the client's home permanently, wherever the client lives. Through 2027 it pays for audio-only sessions with no extra conditions. From January 1, 2028, audio-only needs a client who can't use video or doesn't agree to it. In 2028 Medicare also starts requiring an in-person visit within 6 months before a client's first mental health telehealth session, then one every 12 months. Clients who start telehealth before 2028 only need the yearly visit. The CMS telehealth page has the current Medicare rules.
